Parental Consent and Travel

Every summer, we notarize quite a few parental consent forms for children or minors who may be traveling with just one parent, another relative, or a friend of the family. They typically include your name(s), contact information, any special instructions, and consent for the child or minor to travel.  They can include the dates or travel, name who they will be with, and give consent for directing medical treatment should the need arise. Templates for this kind of consent are available from many primarily online sources but we are not allowed to provide the consent forms/letters as notary publics.

Once you have the document in hand, we can set an appointment and we’d come over to conduct the notarization.  You would just need to print the document and have an unexpired license or passport with the signing parent when we meet.  If the parents aren’t living together, we are happy to print and bring the document to them without you for the notarization. Typically, we’d provide an attachment acknowledgment or jurat to the consent that would be kept with the signed document while traveling in case it is needed.
